Rental Agreement
I, as the renter of the listed garments (merchandise) either on my "In-Home Fitting Form", Online Account or Individual Order Form will be responsible for the safe return of said merchandise. The merchandise is due back the following Wednesday after the affair date. After that date, I authorize JRFormalwear to
bill me $20 per day late fee. After two (2) late, I am responsible for the full purchase price of said merchandise. Any changes to the tuxedo two (2) weeks prior to the affair will result in a $10 handling charge. A cancellation fee of $30 will apply if cancelled within two (2) weeks of the affair. Tuxedo rental must be paid in full one (1) month prior to the affair date or risk being cancelled.
All client information given is true and accurate and acknowledge my submission of this order is binding to this agreement.
If you have submitted any order via E-Measurement, Phone-In or Individual Order and your measurements are incorrect a
shipping fee of $5 will apply.

DAMAGE WAIVER
Renter agrees to pay the sum of $5 for the duration that the rental agreement is in effect, and JRFormalwear agrees to waive claims against renter for MINOR damages to merchandise provided damage was not due to negligence or reckless abuse and is repairable.
